Outpatient CT Referral Form

We are happy to accept referrals for outpatient CT scans at our surgery in Sherborne, which we operate Monday to Friday each week. Unless otherwise
requested, images will be sent to VetCT for interpretation and these reports will be forwarded to the referring vet, who will be responsible for all follow-up care.

Referrals can only be made by veterinary surgeons; if you are a pet owner wishing to refer your pet for a CT scan, please talk to your veterinary practice about this.

As a local practice referring to one of our peripatetic or CT services, we would like to reassure you that we will not register your referred client for first-opinion work with us for the following 12 months.

*Please note that we will only consider direct insurance claims with a completed pre-authorisation, which we are happy to assist with.
